For COVID-19 Positive Individuals with Disabilities Who Live in Community-Based Settings


The following diagram depicts how individuals living in community-based settings who tested COVID positive may face disruptions in services and displacement from their current location. Each step should be explored with both the CMIST Framework and person-centered planning at the forefront, and options should be freely chosen by the individual whenever possible. They may need support from people they know and trust and the people providing services and supports need to work to accommodate these preferences.

Living in a community-based setting and screened because of COVID exposure or symptoms:

  • COVID negative
  • COVID positive
    • Based on screening, testing, and a review of the individual's person-centered plan, the individual is triaged and may be moved to temporary shelter, medium/high acuity alternate care sites (ACS), or a hospital
      • Uninfected (false positive)
        • Return to original community-based setting
      • Receiving treatment (Duration of illness)
        • Deceased
        • Recovered
          • The individual is cleared to leave isolation or quarantine but a) may need assistance to return home or b) must first be disgraced to a temporary care setting to receive lower acuity care; the person-centered plan should be reviewed.
            • Return to original community-based setting
            • Unable to retrun to original community-based setting
            • Enter a new community-based setting.
